Thank you for bearing with us with all of the consents we are required to obtain at check in. As you may have noticed, we have added a check-in secretary to help you through the process. 

Why all the consents? 

Privacy laws require us to have your permission to leave appointment and medical information on your home phone, mobile, text  mail, email or with your emergency contact person. 

We need Health Care Proxy forms to know who has your permission to make your medical decisions if you are unable. 

We have 3 information exchange consents for you to sign.  The first two are the Buffalo and Rochester area hospital, lab and radiology facilities.  If you happen to go to any of these locations for any reason and they don't automatically send us a report, this gives us permission to obtain those reports.  The third one is Surescripts.  If you go to a specialist and they give you a new medication, but you can't tell us what the name or dosage is, this gives us the permission to download your prescription history through your pharmacy.
